Automate appointment reminders and follow-ups for a clinic in Quebec

In short

A clinic can hand the AI its appointment confirmations, day-before reminders, waitlist backfill when someone cancels, post-visit follow-ups and recall reminders for hygiene or annual exams. Messages go out by text or email, your staff keeps the final say, and every send is logged.

Calling is the shortest path. By form or by email, we reply within 1 business day. The audit takes 30 minutes and commits you to nothing.

The problem is described by the profession’s own regulator

In February 2025, the Collège des médecins du Québec published its figures on missed appointments: more than 400,000 medical appointments missed every year in Quebec, out of roughly 18 million scheduled. Among family physicians alone, about 30,000 appointments are missed each month.

The Collège is not asking patients to do better. It is asking for a simple way to cancel or reschedule an appointment — and in private clinics, it places that responsibility on the owners. In other words, the regulator itself describes the service to put in place.

Those figures are from February 2025 and give the scale of the problem in Quebec, not the scale of yours. You already have your own number: take the empty slots in a normal week and what one appointment is worth to you. If an appointment is worth $150 at your clinic, multiply the two and you have the amount at stake every week.

What “appointment reminders and follow-ups” actually covers

It is not one task, it is five. We put them in place one at a time, starting with the one that hurts most.

1

Appointment confirmation and reminder

The patient gets a confirmation when the appointment is booked, then a reminder before the date, by text or email. Confirming, cancelling or asking to reschedule takes one action. That is exactly the simple way out the Collège is asking for.

2

Filling cancellations from the waitlist

A cancellation frees a slot. Patients on the waitlist who match that slot are contacted in the order you set, and sending stops as soon as someone accepts. The slot fills without anyone picking up the phone.

3

Post-visit follow-up

A follow-up message after the visit: instructions, next step, an invitation to reply if something feels off. Replies that call for clinical judgment are routed to the right person, never handled by the AI.

4

Recall reminders (hygiene, annual exam)

Patients due back in six months or a year are reminded at the right time, with the reason for the visit. It is the task that gets pushed aside on a full day, and the one that fills the agenda in slow weeks.

5

Chasing balances and claims

Unpaid balances, claims to complete, missing documents: the reminder goes out with the amount and what to do next. Escalation to someone at the clinic follows the rules you set.

Work out what your empty slots and manual follow-ups cost you

The five numbers below belong to a plausible clinic, not to yours. Replace them with your own figures: the picture is redrawn with every keystroke.

One calculation convention, stated plainly: a month counts 4.3 weeks. Every other number is one you entered above — nothing is assumed elsewhere.

Based on your numbers, here is the order of magnitude

  • Slots left empty per month

    51.6 slots

12 slots per week × 4.3 weeks per month = 51.6 slots per month

  • Revenue those slots represent

    $7,740 per month

    51.6 slots × $150 per appointment = $7,740

  • Hours per month spent on follow-ups

    11.5 hours

    40 follow-ups per week × 4.3 weeks × 4 minutes ÷ 60 = 11.5 hours per month

  • What that time costs

    $322 per month

    11.5 hours × $28 per hour = $322

  • What the current situation costs you, per month

    $8,062

    $7,740 of empty slots + $322 of follow-up time = $8,062

Filling every slot is not the point: filling only some of them already changes the picture. Out of the 51.6 slots left empty this month, each one taken back is worth $150 at your clinic, and every follow-up you no longer do by hand takes time out of the 11.5 hours above.

These amounts are an estimate computed on the numbers you just entered. It is not a guaranteed result, and we are not announcing any amount recovered: the calculation describes the current situation as you described it.

The calculation stays in your browser: nothing is sent, nothing is stored, nothing is measured.

A typical day, step by step

Here is how a day runs once the system is in place. Every step is labelled: done by the AI, or kept by your team.

  1. Early morning AI

Reminders for tomorrow’s appointments go out by text and email. Confirmations that come back are written to the schedule.

  2. Mid-morning Your team

The receptionist opens a short list: only ambiguous replies and reschedule requests outside your rules are waiting on a decision.

  3. Midday AI

A patient cancels. Waitlist patients who match the slot are contacted in the planned order; the first to accept gets it, the others get a closing note.

  4. Afternoon AI

Yesterday’s post-visit follow-ups go out. Replies that mention a symptom are flagged and handed to the clinician.

  5. End of day Your team

The owner reviews the day: reminders sent, confirmations, slots refilled, balances chased. A rule gets adjusted if needed.

What about your clinic software?

Clinics in Quebec often work with Dentitek, ClearDent, Medfar/MYLE, Omnimed or Progitek.

  • Dentitek
  • ClearDent
  • Medfar/MYLE
  • Omnimed
  • Progitek

A connection is generally possible as soon as your software offers a connection interface; we verify this during the audit. We promise no integration before that check. If there is no interface, we look at exports, dropped files and assisted entry, and we tell you plainly what that changes for the project.

Law 25: what it means for a clinic

An appointment reminder carries health information. Here is what matters, in plain language. This is not legal advice: your own legal counsel stays your reference.

  • Health information is sensitive information: using it requires clear consent from the person concerned.
  • Since September 22, 2023, a clinic must have a personal information protection policy and a person in charge of protecting that information, whose contact details are published.
  • Putting in place a system that processes personal information, or communicating that information outside Quebec, requires a privacy impact assessment.
  • A confidentiality incident that presents a risk of serious injury must be reported to the Commission d’accès à l’information and to the people concerned.

What we deliver with the system

  • The privacy impact assessment for the system we put in place.
  • The processing register: what data, for what purpose, for how long.
  • The list of subcontractors and the hosting region of every supplier.
  • The name of the person in charge of protecting personal information, to carry into your policy.

Your clinic’s professional order may add requirements of its own, on top of the law. We account for that during scoping.

Timeline

First process in production in 2 to 6 weeks

A project runs between CA$4,000 and CA$25,000. Simple reminders by text and email, with one-tap confirmation: low end of the range. Several systems connected — clinic software, waitlist, billing: high end.

The first process is in production in 2 to 6 weeks. We put one in place first, then add the others.

And the phone ringing meanwhile?

Reminders and follow-ups go out by text and email. If your real problem is the phone ringing with nobody to answer — booking by phone, the lunch-hour rush, evening calls — that is a different job, and it has its own site.

Frequently asked questions

Can the patient cancel or reschedule on their own?

Yes, and that is the heart of it: the reminder message carries a simple way to confirm, cancel or ask to reschedule. That is precisely what the Collège des médecins du Québec called for in February 2025. The cancellation rules stay yours: minimum notice, accepted reasons, fees if you charge any.

Do messages go out by text or by email?

Both, depending on what the patient agreed to. Text is usually read faster; email can carry a document or a form. The channel and the send time are settings, not a technical constraint.

Will it connect to my clinic software?

A connection is generally possible as soon as your software offers a connection interface; we verify this during the audit. We promise no integration before that check. If there is no interface, we look at exports and files, and we tell you plainly what that changes for the project.

Is this Law 25 compliant?

The system is built for it: clear consent, data hosted in Canada, a privacy impact assessment, a processing register and a list of subcontractors handed over at delivery. The clinic stays responsible for its policy and its designated person in charge; we provide what you need to keep both current.

How long before I see a result?

The first process is in production in 2 to 6 weeks. The result is measured on your own numbers: empty slots per week before, empty slots after, and slots refilled from the waitlist.

Does the AI talk to patients unsupervised?

No. Messages follow templates you approve. Anything outside the frame — a clinical question, a complaint, a delicate situation — is handed to someone at the clinic. Every send is written to the log.
